What is home health care?

Home health care involves a vast range of services that can be delivered in your home. Whether you are recovering from injury or managing illness, the care we provide is just as effective as the care you would receive in the hospital setting, with the added benefit of convenience in receiving care in the comfort of your own home.

Who is eligible for home health care?

To be eligible for coverage of home health services, you must have a need for intermittent or part-time skilled services, and you must be “homebound,” meaning:

  • Because of illness or injury, you have difficulty leaving your home without assistance (e.g., using a walker, cane, wheelchair, crutches; help from another individual; special transportation)
  • You have a normal inability to leave your home because it requires a taxing effort
  • Due to your condition, it is not recommended that you leave your home


What if I need help after hours?

We provide 24/7 support in case of any emergencies. Even if you call our office number, (847) 982-9862, outside of our hours of operation, a member of our team will always be available to address any questions and concerns.

How often will a nurse or therapist visit me?

The frequency of visits by your nurse or therapist will be determined by your individual care plan, which is developed by your primary care provider and our home health team.

Will my insurance cover home health care?

Most insurance plans, including Medicare, cover intermittent home health care services if certain eligibility criteria are met, including being “homebound” or having needs for skilled services. We will help to verify your insurance coverage and benefits before services are provided.

How is my care coordinated with my doctor?

Our team will regularly update your doctor on your progress and any changes in your condition. We work closely with your doctor to adjust your care plan as needed.
